I also need another retrieve for other forms and I have codes but when I click retrieve only the retrieve data was appear I want is the retrieve data will appear also in the same form.

I've not read it, but of you are using if else switches, this can be the culprit.
You're displaying the form after else and the retrieved data after if isset or something else.
